3. Lost Physical Share Certificate: Apply for duplicate certificate; company issues Letter of Confirmation; deposit for Demat.
4. Shares in Inoperative Demat Account: Provide required KYC to the Depository Participant to reactivate the account.
5. Shares transferred to IEPF Authority: File IEPF Form-5 online on IEPF portal, and submit physical form to Infosys Limited.
